Multi-Site Medical Groups · MSOs · Specialty & PT/MSK Platforms
The margin in a medical group sits between the referral and the payment.
Multi-site medical groups, MSOs and specialty platforms rarely lack demand. They lose it to slow access, leak it through referrals that never get scheduled, and give revenue back to denials and rework, while each acquired practice brings its own systems and habits. We baseline access, revenue cycle and integration performance across your sites, redesign the work, and build the automation and AI that hold the gains. Clinical decisions stay with clinicians.

The landscape
Consolidation built the platforms; the workflow still decides performance.
Physician practice has consolidated quickly. As of January 2026, 82.0% of US physicians were employed by hospitals or other corporate entities, according to research by the Physicians Advocacy Institute and Avalere Health. For PE-backed groups and MSOs the thesis rests on scale: a shared revenue cycle, central access, better payer leverage. In practice, many platforms still run each acquired practice its own way, and the synergies remain in the integration plan rather than the P&L.
Meanwhile the work around each visit keeps growing. The AMA finds practices complete an average of 39 prior authorization requests per physician per week, taking physicians and their staff an average of 13 hours. In a March 2024 MGMA Stat poll, 60% of medical group leaders reported rising claim denial rates. At the front end, AMN Healthcare puts the average wait for a new-patient appointment, across six specialties in 15 large US metros, at 31 days. Slow access and revenue leakage usually share a cause: manual work, done differently at each site and measured too late.
The automation opportunity is well documented. The CAQH Index estimates a remaining $21 billion savings opportunity from fully automating manual and partially manual administrative transactions. We apply AI where it removes that work, in eligibility checks, authorization packets, referral intake, scheduling and denial workqueues, and leave clinical and coding judgment with clinicians and coders. Where we handle protected health information we sign a BAA, and we scope data access in the diagnostic.

Where performance is lost
Access is the growth constraint
New patients wait weeks for a slot while templates run below capacity and cancellations go unfilled. Demand goes to whoever offers the earlier appointment.
Referrals that never become visits
Referrals arrive by fax, portal and phone, sit in queues, and are rarely closed back to the referring office. Leakage is seldom measured, so it is seldom managed.
Denials and prior authorization tax every visit
Eligibility, authorization and registration errors surface weeks later as denials and rework, and the cost lands on staff who are already stretched.
Acquisitions that never fully integrate
Each tuck-in brings its own practice management system, payer contracts, fee schedule and front-desk habits. The platform reports one EBITDA and runs as many operating models as it has acquisitions.

ACCESS
Patient Access & Referral Performance
Shorter time to appointment and more referrals converted into visits, with scheduling, intake and referrals run as one managed flow across sites.
What we measure
- New-patient lag and third-next-available appointment
- Referral-to-visit conversion by source
- Template utilization and no-show rate
01
Central scheduling and contact center redesign
Scheduling rules, templates and call handling standardized across sites, with automated agents handling routine booking, rescheduling and reminders while staff take the complex calls.
02
Referral intake and closed loop
Referrals captured from every channel, checked for completeness, authorized where required and scheduled, with status reported back to the referring office.
03
No-show recovery and backfill
Reminders targeted by risk, cancelled slots backfilled from the waitlist, and outreach to patients overdue for a follow-up their clinician has ordered.
MARGIN
Front-End Revenue Cycle & Denial Prevention
Fewer denials and faster cash, by fixing eligibility, authorization and registration before the claim and working what still denies through prioritized workqueues.
What we measure
- Clean-claim rate and initial denial rate
- Days in A/R and cost to collect
- Prior authorization turnaround and staff hours
01
Eligibility and benefits at scheduling
Coverage, benefits and patient responsibility checked automatically when the visit is booked and again before it takes place, with exceptions routed to staff and estimates ready for the front desk.
02
Prior authorization preparation
Authorization requirements identified at order, packets assembled from the chart and submitted through payer portals, status tracked, and clinical questions routed to the clinician.
03
Denial prevention and workqueues
Denials categorized by root cause and payer, fed back to the step that caused them, and remaining work prioritized by value and timely-filing risk.
VALUE CAPTURE
Acquired-Practice Integration
Each acquisition reaches platform performance sooner, on one operating model, with the synergies in the investment case tracked against a baseline.
What we measure
- Time from close to KPI parity with the platform
- Days from provider start to first billed claim
- Synergies captured versus the investment case
01
Standard integration sequence
A repeatable plan for each acquisition covering payer enrollment, system migration, central scheduling, revenue cycle cut-over and staffing, with a named owner for every step.
02
Credentialing and enrollment tracking
Provider credentialing and payer enrollment tracked by provider and payer, with documents gathered and submissions prepared automatically so new clinicians can bill sooner.
03
Synergy and KPI tracking
Every site measured on the platform KPI set from close, so management and the sponsor can see which synergies are captured and which are still on paper.
The diagnosis
What we look for in your practice.
A Performance Diagnostic measures these in your own systems first, so every later result is judged against the same baseline.
New-patient lag and third-next-available appointment by specialty and site
Template utilization, no-show and late-cancel rates, and how open slots are backfilled
Referrals received, scheduled and completed, by referring source
Clean-claim rate, initial denial rate by reason, and days in A/R by payer
Prior authorization volume, turnaround and the staff hours behind it
Point-of-service collections and patient balance aging
For acquired practices: time to payer enrollment, system migration and KPI parity with the platform
How an engagement runs
01
Performance Diagnostic, 2 to 4 weeks
A fixed-fee diagnostic. We pull scheduling, practice management and clearinghouse data across sites, baseline access, referral and revenue cycle performance, and return three priced improvement plays with a 90-day plan. Sites are compared on the same definitions, often for the first time.
02
The first 90 days: First play live
Typically central scheduling and referral intake, or denial prevention at the front end. We redesign the workflow with site and revenue cycle leaders, build the automation into your PM/EHR and clearinghouse, and report weekly against the baseline.
03
Then: Standardize and integrate
Proven plays become the platform standard and the integration playbook for each new acquisition, with one KPI set that management, the board and the sponsor can audit.

Yes. Where our work involves protected health information we sign a business associate agreement, and we scope in the diagnostic what data we access, where it is processed and what stays inside your systems. We answer your security review directly and do not claim certifications we do not hold.
No. We work on scheduling, intake, referrals, authorization preparation, billing and reporting. Clinical judgment, orders and documentation remain with your clinicians, and coding decisions remain with your coders.
Often more so. Many denials start at scheduling and registration, which a billing vendor does not control. We baseline the vendor's performance alongside your front end and help you manage the relationship on clear KPIs.
No. We baseline across the systems you have, and integration plays often include the migration sequence. Many improvements, such as central scheduling rules and denial feedback loops, work across systems from the start.
Yes, tuned to the model: plan-of-care completion, visits per case, cancellations, authorization and visit limits, and therapist utilization across clinics. The diagnostic and the engagement shape are the same.
